Are there examples of widespread no right turn on green rules in any jurisdiction? If it's right turn on red only, cars still have pedestrians crossing in front of them to deal with, and then they just end up blocking the crosswalk.

Technically feasible? Maybe, but it's an awful idea from a city building perspective. Adding more cars to the downtown core is the worst thing we could possibly do. The road networks downtown is likely at or over capacity as it is, and then you also need to significantly expand parking...

The TTC font has been in all design publications I've seen. Expect the whole thing to look similar to the TTC subway system.
