I wouldn't say it's very badly designed. I've seen far worse. Is it any worse than the long walk do the GO terminal at York Mills? And far better than the Line 1/2 transfer at Spadina. Just the walk from the closest ticket booth to Line 1 at Spadina was absurd.

The difference between Kennedy and the other transfers you mentioned is that the transfer at Kennedy is a transfer to continue on the same journey you were already going. The Scarborough RT was built to be a cheap continuation of Line 2, as a result a ton of people use Kennedy for that specific purpose. The transfer at Spadina and York Mills are comparatively not a big deal since the spadina transfer really only serves those transferring between the 510 streetcar and the Yonge line (those transfering to Line 2 are usually going to transfer at St. George), and York Mills GO bus isn't that big of a GO bus terminal. It ultimately falls down to the type of traffic you're expecting from your transfer. To use a non-Toronto example, the Mascouche Line in Montreal used to operate out of Gare Centrale through the Mont Royal tunnel, operating on the same trackage as the Deux-Montagnes line, however since the tunnel is being converted to the REM, they needed some way to get the Mascouche Commuters to downtown, and their solution was to create a new interchange at Cote-de-liesse between the REM and the Mascouche Line to act as a terminus, and have a cross platform connection. While its not the most elegant solution, its only acceptable because the transfer is quick, easy, and painless. Anything less, even if it was 2 island platforms one for each service would've been met with backlash and criticism. Its a very similar story with Kennedy where the majority of people that get off at Kennedy transfer to Line 3 to get to Scarborough Center. Because of the traffic of people transfering, the transfer needs to have a higher standard in terms of what's acceptable, and the current solution simply doesn't cut it.
 
Oh god, not the "we deserve it" argument again. They deserve reliable fast transit, but there's no destination east of Fairview Mall that really deserves to be the Line 4 terminus! Transfers suck, but that should be fixed by changing the existing technology on Line 4 to something we can easily extend.

Except for the Toronto Zoo, colleges (IE. Centennial College , University of Toronto Scarborough (UTSC), etc.), and three hospitals. Humber College's North Campus and William Osler Health System - Etobicoke General Hospital has Humber College Station at the terminal station for the Line 6 Finch West LRT.

I'm pretty sure none of those are on Sheppard. It would be better to have the line swing south to meet Scarborough Centre and Centennial College, before swinging up along Progress and the old rail corridor to terminate at Malvern, but that's really not practical with Line 1's rolling stock.

I'd think any transfer would be to continue an ongoing journey.

I'd be surprised if the number of daily transfers from Line 1 to Line 2 and the streetcar at Spadina station are fewer than those changing between Repentigny commuter train and the Deux-Montanges line.

Its not about "ongoing journey" rather is it a transfer that almost everyone is going to take (What percentage of commuters will continue in the same direction?). When you get to Kennedy, you don't have a choice to continue East on Line 2, the only thing you can transfer to is GO which isn't the most widely used service, and in general most people will move to LIne 3 which travels in the same direction as Line 2 (The North East direction). Same thing with the Repentigny trains where the traffic might be comparable to Spadina, but the ratio of passengers taking the transfer is somewhere around the 100% mark, not to mention that you're trying to compensate for what was at one point, a direct through service.

Vic Park has a ton of employment and general density, further to Agincourt and its the same story (plus a load of redevelopment) and once you get to Agincourt might as well go to Scarborough Centre.
